  • The cost of base medications and associated shipping costs

  • Lab phlebotomy fee if labs are collected in our office

Not included:

  • Adjunctive medications (anything listed on the a la carte menu)

  • Up charges for certain special medication formularies, i.e testosterone cream rather than injection for men

  • Lab fees which will vary based on the panel needed

  • Lab phlebotomy fee if drawn outside our clinic (typically $10-20)
